Ambulance services are the same as hospitals ......... what does the data say ?

In the midst of a very public audit of Ambulance services in Victoria, I thought it may be useful to reflect on some of the issues facing those of us working with ambulance data, and to highlight the similarities and differences between that data and the data captured in the hospitals that ambulance services may be compared with (I will leave you to draw your own conclusions regarding the wisdom of such comparisons)

How about we think about just the emergency component of overall ambulance service provision, and how most work is generated as a starting point. This typically begins with a call to an independent external agency responsible for receiving calls using yet another company’s software product........... this data is then forwarded on to the ambulance service.

A further complexity at this point is that “b” different bystanders may contact the external agency about a single event – let’s say a car crash. What may not be clear at that stage is how many patients are involved – let’s call that number “p”. At that point an educated reader like yourself would recognise a difference with the way hospital work is “instantiated” into business systems and processes.

Through an internationally standardized process a decision is made about appropriate resources ......... and that may be several............to send to the scene.  What precise resources (“r”) are sent is a complex decision .......... remember – ambulances don’t just sit at a station waiting to be called .........oh what a luxury!!!

In another entirely  appropriate process, “r” resources may then be changed (up or down)  to “r-new” resources as more information comes to hand (and remember that each of “r-new” may be a different type of resource depending on the clinical scenario – there may be “v” variants of resource type). Ohhh...... and don’t forget that “a” numbers  of paramedics may arrive to treat “p” patients – as not every ambulance  vehicle is configured in a 1 vehicle - 2 paramedic configuration

Depending on the clinical scenario and the value of “p” .............”t” numbers of people may be transported to hospital and “s” numbers of people  may be treated at scene ................I think you are getting a picture of the differences with hospital  data by now !!! 

Let’s not forget however, that all of this may happen at any one of “l” (a very large, nearly infinite number of locations) across the state........... and that “l” is a partial determinate of the value of “r” and “r-new”....... and appropriately so as there are clearly not “f-r” and “f-a”(fancifully  large numbers of ambulance resources, and paramedics respectively) funded to sit around  and wait to respond to every permutation and combination of the variables above

There........... I think that explains it in simple terms  !!!!  You should now be able to see how really hospital data (1 patient per episode of care, usually within 1 hospital (itself in 1 fixed location), only ever in 1 ward at a time) is quite different to ambulance data ..............now if only I could convince those  who know better J

Have we missed an important issue in the reform discussion   (published in Digital Healthcare Design)

I think that the current debates around improving healthcare for all Australians could have missed a very important point.

Personal electronic health records will have a positive, but long term outcomes, certainly the hospital access issues dealt with in the reform package are important and of course, adding more hospital beds will provide some short term relief.  However, none of these initiatives fully address the really pressing need to transform the biggest systemic issue in health, the cost and quality of care within Australia’s public hospitals.

Right now, Australian governments are funding or planning to fund over $33 billion worth of public hospitals which are in the design or construction phase.  With hospitals representing the single biggest component of our recurrent healthcare expenditure ($30.8 Billion in 2008/2009) it is crucial that care professionals and consumers alike focus on how we build better and more efficient hospitals. 
 
This issue is both important and urgent; the hospitals we are building now will frame the way we deliver care for the next 20 years and define how we cope with onslaught of our aging population. 
 
By designing smarter, more patient focused healthcare facilities we can make a profound impact on the millions of Australians who come through these institutions in the near term. We can really impact the sustainability of our healthcare system.

Building common ground between design, delivery and IT in health   (published in Digital Healthcare Design)

I thought I should spend some time explaining my last blog post. 

My intent in putting together the model was to start the process of defining where the common ground is between those who design and deliver the built environment of a healthcare facility and those who produce the information systems, on which the care, social and operational processes which occur within those institutions are based.
 
The reason I have put my thoughts forward on this is a result of the overwhelming change in relevance for information systems in care delivery.  The transformation that has occurred over the past 10 years has taken information systems from being and an important adjunct to the operation of a hospital to the current situation where they are now a critical element of the care process.
 
As a result we need to start thinking more closely about how these information systems and process interact with, and leverage from, the environments in which they are used.  
 
To do this my thought process has been to identify our common objectives.  In this case the concept of People Centred Care sits at the centre of all our activities. This concept sets the delivery of care as a collaborative process set between the care providers, the patients and their supporting community, delivered at the level appropriate to the needs and skills of the people involved. 
 
The next layer in the model defines that the tenants of people centred care need to be observed in all the four major functions of a hospital, that is, the Clinical, Operational (support services) Education/Research and finally the Social aspect, that is, how a hospital supports the communities that operate within and around it.
 
The final layer of the model identifies the outcomes we want to generate in each of the functional areas, some of which are more or less relevant, depending on the functional area in question.  This is where I hope we can bring the design, delivery, clinical and health IT teams together to drive the discussion around what the important issues are and how the design and information systems can work together to deliver a more synergistic, more effective outcome.
 
My thought is that it is only when we define some specific areas for interaction do we focus down and really consider what the important activities are and how we can come together and improve delivery.
 
So really this is all about setting up a straw man to drive discussion, from my perspective, it doesn’t really matter what any model actually looks like, so long as it enables us to start the discussion, that is the important issue.  Once a broad based discussion has started the combination of intellects involved will guide us to a productive outcome.

The intersection between the built environment and IT   (published in Digital Healthcare Design)

I have further developed the Digital Hospital Design workshop discussion model that I mentioned in an earlier post . The objective was to add a set of common outcomes that both the information technology and the built environment communities design to.  This is my proposal for the areas of intersection between our communities. I have added some definitions below the diagram for added clarity.
 
My longer term intent is to encourage discussions around defining areas of common endeavour where we could improve our understanding of each other’s capabilities and so build the communication and synergies between all the communities involved in the delivery of a hospital or healthcare facility.  I see the workshop in a few weeks time as a significant step in this process (see www.digitalhealthdesign.org )
 
 
 
 
Understanding
This encompasses both the concept of effective communication, that is the delivery of information in the appropriate format, in the right environment to enable the efficient transfer of knowledge, as well as the social concept of empathy that is inherent in really comprehending other perspectives.    Both aspects of understanding are important in clinical environment.
 
The transfer of knowledge and the creation of empathy are both highly dependent not only on the way the information is structured, but also how it is delivered including the physical characteristics of the environment in which it is delivered.  Is the space where communication is occurring quiet and without distraction, is it a space where people can focus their attention.
 
Community
This is the ability to interconnect people with a mutual interests in such as way as they build network connections (real and virtual) with people that may enrich their experience in way that they feel is rewarding.    Importantly, for the care environment, both the social and the clinical/analytical aspects of building community are important to the care process
 
Teamwork
Bring together care providers, patients, their families and relevant parts of the broader community to work together to deliver care and support persistent positive lifestyle changes.
 
This is a critical element in the delivery of people centred care.
 
Privacy
Much of health is dependent upon the ability to both share what is need and keep private what is not.    This balance needs to be under the control of the information owner, whether patient, clinician or healthcare manager and is highly dependent not only on the information system which contains the data but also on the physical environment in which the information is shared.
 
Effectiveness
The rapidly increasing demand and escalating cost of care means that the operational effectiveness of a facility is of critical importance in its design.  This cover all levels of how to support work practices and processes that define best practice in the ways a hospital can deliver upon its Clinical, Operational, Education/Research and Social responsibilities.
 
Relaxation
The ability to provide an environment which minimises stress, for all those involved in the care process (patients, staff, family and community) is a key element to enabling effective care and efficient operation of the healthcare facility.

Hospital Guidelines, The Digital Hospital Design Workshop and Crocodile Handling Standards   (published in Digital Healthcare Design)

There are two major objectives of the Digital Hospital Design Workshop:

  • Drive innovation in hospital design through bringing together Health IT with Architects, Designers, Engineers Health Managers and Clinicians
  • Establish a core group of individuals who are interested in developing a set of guidelines for the application of Health IT in Hospitals.

Currently the Australasian Health Facility Guidelines, which are owned by the Health Infrastructure Alliance (AHIA), on which each of the jurisdictions has a representative, contains almost no details on the application of Health IT in hospitals builds.  The consequence of this is that when the final value management stage of a hospital tender is reached and a decision has to made as to what is included in the facilities build, Health IT often misses out or its capabilities are greatly diminished.   The impact of this can be hospitals which do not have the capability to take advantage of current or future information technologies and/or require expensive modifications.


With over $30 billion of hospitals being built just on the east coast of Australia at the moment, there is a pressing need for the creation of guidelines for the inclusion of Health IT in Hospitals.    These are the hospitals which will govern our care over the next 15 to 20 years, so we have to make sure they are appropriately designed.


In my discussions with people interested in this area I was made aware the "Safety Guidelines for
Working with Crocodiles in Captivity" and the attached comment "If we can establish detailed guidelines for handling crocodiles ( which mostly consist of the advise for keeping away from the end with the sharp and numerous teeth...see diagrams below)...then why can't we get together some guidelines for something as broadly important as the inclusion of Health IT in hospitals". 

 


My response was to empathise with their position and urge them to attend the workshop.
